Select the artists you would like to have participate in a Sequence Evangelism Concert.

Most concert series include three to four dates, spanning 9 to 15 months. You may even wish to consider multiple artist concerts as well.

List the name of the VENUES you hope to use for the concert series. Inlcude seating capacity, street address and web address if available.
List the tentative dates or months that you are wanting to have concerts.

You don't have to have confirmed dates with your venue at this point. After you submit this form, start calling the venue options you have and check their available dates.

Name of the primary local church that will be managing the concert series.

(Other area churches may be included later in planning advertising, financial support, etc.)

Please include full mailing and street addresses along with any web site address. Other area churches that may join as a co-sponsor may be added later.
